Position: Assistant Coordinator

Where: CBI Home Health - Hamilton / Fennell
Description: Part time After Hours Coordinator required. Specific Duties and Responsibilities • Receives and processes client orders from portal, fax, telephone referral methods. • Organizes and determines client needs. • Determines and schedules appropriate staff reflecting skill, geography, availability and timeliness. • Ensure staff/client scheduling to provide continuity and consistency of care. • Maintains computer schedule to reflect current/actual service requirements. • Documents and changes through procedures any changes/cancellations/new orders/discharges/on holds. • Ensures that offers for all disciplines on the Portal are dealt with in a timely manner in accordance with CCAC policy. • Ensures that referrals are sent to the appropriate discipline via fax and/or email in a timely manner. • Receives and returns phone calls to CBI staff, clients, and CCAC in a professional and timely manner. • Maintains record of staff utilization and unfilled shifts. • Maintains accurate and up to date data in clients Alerts with dates related to communicable diseases, DNRs, and personal reminders related to client. • Abilities to retrieve multiple messages and to follow up in a timely manner. Qualifications/Requirements • College diploma in business/communications an asset. • Understanding of medical terminology and Community Health. • Ability to operate computer with Windows XP, Word, Excel, Internet Explorer and other systems. • Excellent verbal, written communication skills/telecommunication skills. • Excellent organizational skills and interpersonal skills. • Client service/customer training background an asset. • Ability to function under pressure and meet changing deadlines and responsibilities.
