Chronic Pain Program
CBI Health Group offers a comprehensive multidisciplinary Chronic Pain Program that is based on a biopsychosocial treatment model, oriented towards helping our clients in the understanding and management of their pain condition, while providing insight and coaching into the behavioural patterns that result.
We offer an individualized approach to restoring function, stamina and confidence to help patients get back to the activities of daily living and work, following injury or other disabling conditions. Our Pain Management Program is educationally oriented with activity-based components designed to provide a new outlook on your disability, while providing coping behaviours and management strategies. The ultimate goal is to maximize your quality of life and functional ability, and to return you to productive living.
Driver Assessment & Rehabilitation Program
The Driver Assessment and Rehabilitation Program is designed to foster safe and confident driving, and provides assessment and treatment services to drivers of all ages. We assess each individual's fitness to drive in a non-threatening, unbiased and objective fashion. Our goal is to help individuals drive safely when it is possible to do so. When driving skills are found to be lacking, our facilities recommend and provide treatment aimed at fostering safe and confident driving.

Ergonomic Assessment
Ergonomics seeks to harmonize the functionality of tasks with the human requirements of those performing them. The CBI team includes highly trained and experienced Occupational Therapists and Kinesiologists with extensive experience working in home, office and industrial environments. All ergonomic assessments are conducted and written reports are based on accepted ergonomic guidelines.
Our range of Ergonomic assessment services include:

Individual Ergonomic Assessments – an individual ergonomic evaluation focuses on identifying risks of musculoskeletal injury for that employee and makes recommendations to reduce that risk. Ergonomic assessments are part of an overall hazard assessment in a comprehensive occupational health and safety program.

Office Ergonomic Assessments – all ergonomic assessments are conducted and written reports are based on accepted ergonomics guidelines such as CSA Office Ergonomic Guidelines. The CBI Office Ergonomic Assessment and report includes: Confirmation of Essential Physical Tasks and Percentages; Ergonomic Measurements & Layout of the Workstation; Identification of Ergonomic Risk Factors; Recommendations (Engineering, Administrative, Behavioural).

Area Ergonomic Assessment – a general ergonomic assessment of an entire work area or a particular workstation identifies the risk of musculoskeletal injury for the entire workforce. Standardized and informal assessment measures are used and wherever applicable, normative anthropometric and strength data from the male and female population are referenced.
Functional Capacity Evaluations
CBI's Functional Capacity Assessment is a reliable, defensible and highly effective tool to objectively evaluate ability and impairment. The Evaluation is a standardized, scientific procedure to determine an individual's ability to perform meaningful physical tasks in a safe manner. Our FCE is customized to answer specific referral questions, providing an accurate, objective measure of your client's functional abilities in comparison to their physical job demands. Our comprehensive testing is tailored to your client's unique situation and your specific referral questions.
Our FCE will compare functional abilities to critical job demands, analyze each client's physical job demands, identify barriers for return to work, test for Symptom Magnification, identify sub-maximal effort, and utilizes evidence-based protocols supported by normative databases.

Our trained physiotherapists perform the musculoskeletal component of the assessment. Our experienced kinesiologists are trained in Functional Capacity Assessments, and conduct the functional portion of the assessment. The tools used during the assessment have some of the highest reliability results in the field. The protocols used rely on the 'thinking evaluator' approach to testing, thereby carrying high validity. Our comprehensive report includes all test information and a summary of the evaluation as it directly relates to your referral questions. The result is a meaningful and decisive recommendation to facilitate rapid and successful case resolution.

Limited Hand Use Program
CBI has developed a training program specifically for people with injuries to one or both hands. Whether the goal is to completely transfer dominance or to maximize function in an injured hand, we provide the necessary skills for successful participation in school programs and chosen occupations.

Interdisciplinary Pain Program
The Interdisciplinary Pain Program is a comprehensive, interdisciplinary chronic pain rehabilitation program for individuals who are at least 6 months post injury, and for whom pain represents a significant barrier to normal functioning. The treatment team is drawn from physical medicine, psychology, occupational therapy, physiotherapy, exercise therapy and vocational rehabilitation. Consultants are also available from other disciplines as required.
Independent Medical Evaluations
Expedited access to qualified medical specialists is critical in the effective management of both occupational and non-occupational disability. An Independent Medical Examination (IME) is a useful tool to objectively address impairment, disability and/or medical rehabilitation issues. The IME is normally administered when there is a discrepancy in the recommended direction of rehabilitation and a specialist's opinion is required to provide appropriate direction. The IME is generally used when an employee has been off work for a period of greater than 12 weeks. The IME is often combined with a Functional Abilities Evaluation.

Kinesiology
Kinesiology is the study of human movement. At CBI Health Group, our experienced kinesiologists work within our multidisciplinary team to help patients address and manage their health and wellness related issues. Our kinesiologists will assess an individual with various injuries and illnesses related to their anatomy, physiology, biomechanics and motor skills. Our certified Kinesiologists are involved in the rehabilitation, prevention and management of disorders; to maintain, rehabilitate or enhance movement, performance or function in the areas of sport, recreation, work and exercise. Through prevention, rehabilitation, and management our kinesiologists can help you return to your daily function.
Kinesiologists services include:

Promoting Health – working with you to enhance your mental and physical condition, fitness level and overall wellbeing.

Clinical Rehabilitation – working with you to evaluate your particular condition/ailment. Once you have been assessed, through clinical rehabilitation, our goal is to get you back to your maximum physical and mental strength.

Ergonomics – working with you in your home or office environment to ensure proper workstation set up. Once evaluated, some recommendations and changes will be made if necessary.

Lifestyle Restoration Program
The Lifestyle Restoration Program (LRP) is a menu driven multidisciplinary program, and is designed to assist individuals experiencing psychological, social and physical limitations which prevent returning to a specified vocational function. This goal-oriented program is individualized to meet the specific requirements of each participant and has a proven track record for fostering timely return to gainful employment and pre-injury activities. To date over 90% of our LRP participants have achieved the integrated goals of all stakeholders including the participant, family physician, payor and LRP team.
Services are comprised of wellness initiatives that span from early intervention to strategies for overcoming a spectrum of complex physical, psychosocial and behavioural disabilities. The Lifestyle Restoration Program can cover a variety of areas including Reactivation Skills, Life Skills, Nutrition Related Services, Exercise Related Services, and Employment Restoration.

Pediatric Program
The CBI Pediatric Program can help children develop the skills that they need to be successful in their day-to-day activities at home, in school, and in other social environments. By using purposeful and motivating activities, occupational therapists help children develop enhanced sensory processing skills, organizational skills, motor planning, visual motor skills and adaptive play skills. Our Therapists can determine specifically in what areas your child, adolescent or young adult is having difficulty.
Areas of assessment could include:

Fine motor – holding onto toys, crayons, cutting with scissors, handwriting

Gross motor – sitting, walking, jumping, running, skipping

Visual motor – throwing and catching, copying shapes, letters or numbers

Visual perceptual – recognizing letters and numbers, shapes

Sensory motor – avoids certain textures, enjoys tight hugs, likes to swing

Self-help skills – feeding self, picky eating, dressing

As well as, attention, concentration, behaviour skills, and social skills
Following a complete assessment the Therapist will discuss and include the parent/caregiver and the child in developing their goals for treatment.
Pelvic Floor Rehabilitation
Our Pelvic Floor Rehabilitation program can help improve the strength of the pelvic floor muscles, which are key to the prevention of urine leakage. A thorough physical examination is first performed, including an internal pelvic exam. This evaluates the pelvic floor muscles as well as other factors which may contribute to your individual situation.
A specific exercise program is then developed for your needs. Exercises are used to improve muscle strength and speed of contraction, especially for those with stress incontinence. Endurance exercises are also taught, to assist with urgency issues. Functional re-education of the pelvic floor muscles can assist with both types of incontinence. If necessary, electrical stimulation and biofeedback techniques may also be considered in the treatment program. In addition, a comprehensive education program has been developed and will be part of your rehabilitation program.

Personal Training
As part of our one-on-one active rehabilitation approach, our team can provide individualized a Personal Training program to assist in the rehabilitation process. Our kinesiologists study human movement, and are able to provide strategies to improve health and performance. Following a comprehensive assessment, your personal trainer will develop an individualized active rehabilitation treatment plan that will guide you through a progressive one-on-one program designed to improve your core stability, strength, endurance and range of motion.
We work with a variety of patients including individuals suffering from motor vehicle accidents, traumatic brain injuries, whiplash, soft tissue injuries, orthopedic injuries, posture assessment and alignment, workplace injuries, Arthritis, Osteofit, cardiac patients or metabolic disorders.

Physical Demands Analysis
A Physical Demands Analysis (PDA) or Physical Demands Profile (PDP) is an objective measurement of the essential physical demands associated with a specific job classification. A PDP is the basic and required component of several programs and services designed to prevent and manage workplace injuries.

Pre & Post Natal Care
Pregnancy and childbirth are physically challenging for the muscles and joints of the spine and pelvis. Postural changes are associated with the growth of your baby in the uterus, causing your centre of gravity to move forward, and ligaments (the structures which support your joints) become relaxed to assist with the deliver of your baby.
CBI Health Group offers a Pre and Post Natal Care program for mothers that can help with issues related to pelvic or lower back pain, bladder control problems or pain or discomfort with walking. Your Physical therapist will perform a thorough physical assessment to evaluate posture, muscle strength and joint position. A more specialized assessment of the integrity of the pelvic floor musculature should be performed by a therapist who specializes in urinary incontinence. An individual treatment program is then developed for you based on your symptoms and assessment findings.

Return to Work
Return to Work Planning
Return to Work Planning may be required to either reduce the duration of a lost time claim or reduce the duration of a modified work program. A Return to Work Plan outlining the employee's capabilities, identifying the RTW barriers and formulating an implementation strategy is presented for review to both the worker and the employer.
Return to Work Assessment
Combining the spinal/peripheral assessment and fitness for duty assessment is a powerful tool for the employer. The spinal/ peripheral assessment provides information regarding the nature of the employee's problem and outlines recommendations for treatment while the fitness for duty portion provides an outline of the employee's current physical abilities. This provides the employer with the opportunity to provide appropriate modified work in conjunction with an effective time limited treatment program.
Fitness for Duty Assessment
A Fitness for Duty Assessment is an ideal assessment to complete the WSIB Functional Ability Form (T901). A Fitness for Duty Assessment is an objective measurement of an employee's ability to perform the essential physical demands associated with his/her regular job.
Sports Medicine & Athletic Therapy
CBI's team of professionals can address the prevention, immediate care, and reconditioning of musculoskeletal injuries. Prevention includes musculoskeletal and postural evaluation, equipment selection, fitting and repair, warm-up, conditioning programs, prophylactic or supportive taping, and adapting to the activity environment and facilities.
Our team can assesses injuries and conditions, uses current rehabilitative techniques, therapeutic modalities, soft tissue mobilization, physical reconditioning, and supportive procedures to promote optimal healing and safe return to an active lifestyle. Therapist work to ensure optimal athletic performance and the prevention of injuries.
Tennis Elbow
Lateral epicondylitis or "tennis elbow" is a chronic condition that affects athletes who perform repetitive forearm flexion and extension, such as tennis, racquet and throwing sports. Repeated microtrauma to the insertion point of the muscles and tendons that extend (back bend) the wrist and fingers causes "tennis elbow". The pain that occurs will be on the outside of the elbow. It is basically inflammation of the extensor muscles of the wrist. Repetitive motions involved in activities such as typing, using a computer mouse, and dealing cards can also cause "tennis elbow".
Risk factors include:

Weak muscles and /or muscle imbalances
-
Overuse or repetitive motions of the wrists, fingers, forearm
-
Improper equipment may aggravate the condition (i.e. incorrect grip size)
-
Poor ergonomics at work and home
-
Mouse and/or keyboard too low causing an extended wrist
CBI offers a comprehensive assessment and treatment program to address the pain caused by tennis elbow, and help get you on the path to recovery.
Whiplash
Whiplash effects and type of injury varies greatly, however most injuries are to soft tissues such as muscles, ligaments & discs. In the past, whiplash injuries were often treated with immobilization in a soft collar which has shown to slow down recovery. The latest research indicates early movement is the key to recovery. Most people experience a reduced level of activity due to resulting pain and stiffness. The first phase of a recovery plan is to reduce your symptoms and the next phase is restoring you to your usual activity level. Treatment can help control your pain while you increase your activity levels. Short term use of pain relieving medications, gentle exercise, and individualized advice have all been beneficial for some patients. What has been shown to help recovery is to stay active, stay at work or return as soon as you are able, and reduce stress.
