Assessments - Ontario

Occupational Therapy assessments may be conducted in the clinic, at home or work setting:

Hospital Discharge Planning

Our Occupational Therapists coordinate with hospital staff to identify specific needs of each patient. A home assessment is completed to determine appropriate equipment and service requirements for a safe discharge home.

Homemaking Needs Assessments

Our Occupational Therapists complete a detailed assessment of a client's functional status as it relates to their home environment. The need for assistive equipment and/or homemaking services are identified and documented.

Home Safety Assessments

Our Occupational Therapists assess a client's ability to function safely and independently within their home. Recommendations for assistive equipment and/or strategies to maximize safety and independence are outlined in detail.

Attendant Care Assessments

Our Occupational Therapists assess a client's ability to perform their self care tasks. Recommendations for assistive equipment and/or attendant care services are outlined in detail.

Wheelchair, Scooter and Mobility Assessments

Our Occupational Therapists conduct a comprehensive assessment of a client's functional abilities and limitations to assist in determining suitable wheelchair or other mobility options. Our therapists are familiar with local vendors and suppliers as well as private funding guidelines in order to enable an informed and smooth decision process.

Driving Competence Assessments

Our Occupational Therapists provide a comprehensive evaluation to determine the driving competence of medically-at-risk clients. Our CBI driver assessment sites are registered with the Ministry of Transportation of Ontario (MTO).

Driving Anxiety Assessments

Our Occupational Therapists evaluate individuals who report increased anxiety or fear of driving, most often related to a motor vehicle accident. A plan is formulated to help clients overcome their fears and resume driving in a safe and competent manner.

Adaptive Equipment/Assistive Technology Needs

Our Occupational Therapists consider a client's functional status and home environment in identifying appropriate adaptive aid/equipment and/or assistive e technology needs. Our therapists are familiar with local community resources in order to identify cost-efficient options that meet the client's needs.

Job-site Analysis including Physical and/or Cognitive Demands

Our Occupational Therapists attend a client's job site in order to identify critical job demands. This information is used to assist with rehabilitation and/or return to work planning.

Ergonomic Assessments

Our Occupational Therapists meet with a client at work, school or home environment to identify risk factors that may contribute to development of repetitive strain injuries (RSI) or Activity Related Soft Tissue Disorders (ASTD). A comprehensive plan outlining identified risk factors, solutions and associated costs is provided.

Functional Capacity Evaluation (FCE)

A 1 or 2 day assessment using standardized tests and observations to identify a client's physical abilities and limitations. An evidence-based approach is utilized to provide a detailed report that meets rigorous standards of validity and reliability.

Cognitive Functional Capacity Evaluation

A 2-day assessment using standardized methodology for physical and cognitive testing and work simulation activities. The client's abilities can be used as a baseline measure or they can be compared with demands associated with pre-injury job demands.

Paediatric and Handwriting Assessments

Our Occupational therapists can determine in what areas your child, adolescent or young adult is having difficulty. Areas of focus include: fine motor, gross motor, handwriting, visual motor, visual perceptual, sensory motor, self-help, attention, concentration, behavioural and social skills.

Life Care Plan/Cost of Future Care Assessments

A comprehensive medical review and evaluation to determine the amount of support and assistance required to attain pre-accident level of functioning. Future medical, rehabilitation, equipment, home modifications, attendant care, homemaking care, childcare, travel assistance that an individual will require for the long term is addressed.
