Occupational Therapy

Ian Lewis, Clinic Manager / Occupational Therapist

Ian joined Park Street in 2008 as Manager and provides leadership and clinical services. After graduation from the University of Alberta his professional experience has included practice in occupational health and safety, ergonomics, occupational rehabilitation and preschool/school age consultation. He has served and continues to serve in a number of roles on non-profit and professional boards. In his role as a leader and occupational therapist he hopes to enable people to do what they believed was not possible and achieve greater performance in life.

Physical Therapy

Lance Kirkpatrick, Physical Therapist

Lance has practiced orthopedic physical therapy for more than 30 years and has practiced at the Park Street clinic for more than 16 years. His extensive experience performing specialized assessments and keen interest in anatomy and biomechanics have made him a “go to” person for specialists and sponsors.

Raeleen Kirkpatrick, Physical Therapist

Raeleen has varied experience in acute care, outpatient orthopedics and community practice with more than 10 years with CBI. She has specialized training in acupuncture and manual therapy with more than 20 years practice. She is the primary services team leader for Park Street, coordinating primary rehabilitation and student placements.

Jordan Ruder, Physical Therapist

Jordan has a keen interest in orthopedics and manual therapy and is pursuing a Fellowship with the Canadian Academy of Manipulative Therapy. Physiotherapists who are Fellows of the Canadian Academy of Manipulative Physiotherapy (FCAMPT) have completed extensive post-graduate education in the area of orthopedics and have achieved internationally-recognized qualifications in hands-on manual and manipulative therapy. Jordan has been practicing with CBI for more than 4 years and regularly mentors new staff and students while pursuing post-graduate education himself.

Andrea Wollbaum, Physical Therapist (RES)

Andrea joined CBI following graduation from the University of Saskatchewan with her Masters of Physical Therapy. She also has an undergraduate degree in Kinesiology. She provides physical therapy – specialized treatments, education, exercise instruction and program coordination – in our primary program in a team of exercise and occupational therapists.

Dayna Onrait, Physical Therapist

Dayna joined CBI in 2012 and has graduated from the University of Saskatchewan with her Masters of Physical Therapy. She also has an undergraduate degree in Kinesiology. She provides physical therapy – specialized treatments, education, exercise instruction and program coordination – in our primary program in a team of exercise and occupational therapists.

Kinesiology

Lynsey Kradovill, Kinesiologist

Lynsey has received her Certified Exercise Physiologist (CEP) designation from the Canadian Society of Exercise Physiology (CSEP) - an advanced accreditation recognizing advanced skills over and above a Bachelor’s degree in Kinesiology. She has practiced with CBI for more than 6 years and has advanced skills in exercise prescription, myofascial release, client coaching and return to work planning.

Christine Loden, Kinesiologist

Christine has received her Certified Exercise Physiologist (CEP) designation from the Canadian Society of Exercise Physiology (CSEP) - an advanced accreditation recognizing advanced skills over and above a Bachelor’s degree in Kinesiology. Her experience spans outpatient orthopedics, community health and chronic disease management. In her practice with CBI she works in our multidisciplinary rehabilitation program helping clients with complex rehabilitation needs through education, exercise prescription and coaching.

Jared Dormer, Kinesiologist

Jared has achieved his Certified Exercise Physiologist (CEP) designation from the Canadian Society of Exercise Physiology (CSEP) - an advanced accreditation recognizing advanced skills over and above a Bachelor’s degree in Kinesiology. As one of the newer members of our team he has been a quick study in orthopedics and exercise prescription as he works alongside physical therapists designing strength and conditioning programs for clients at all stages of rehabilitation. His clients will range from acute strains and sprains to complex post-surgical repairs.

Chiropractic Care

Dr. Donald Kelm, Chiropractor

Dr. Kelm provides biomechanical treatment in our multidisciplinary rehabilitation program. He has an undergraduate degree in Kinesiology and over 25 years experience as a chiropractor. He combines these two disciplines to deliver well-rounded care through a variety of myofascial and biomechanical treatments.

Psychology

Michelle Shaw, Registered Doctoral Psychologist

Michelle has her Approved Practice Endorsement as a Registered Doctoral Psychologist and Neuropsychologist and has been practicing with CBI on a full-time basis since 2004. As a psychologist her primary focus has been the effects of anxiety and mood disorders on return to work and productive roles. She maintains a special interest in post-traumatic stress disorder. She provides psychological treatment on an out-patient basis as well as for clients in multidisciplinary rehabilitation programs. She also provides specialized neuropsychological assessment.

Pamela Clarke, Registered Doctoral Psychologist

Pamela has her Approved Practice Endorsement as a Registered Doctoral Psychologist. She has been practicing with CBI for more than 2 years and provides specialized treatment and assessment. She draws upon experience in community mental health and rehabilitation and sees outpatients and clients in our multidisciplinary rehabilitation programs.

Intake

Nicole Singh, Intake Coordinator

Nicole is available to answer sponsors’ and customers’ questions regarding our specialized assessment services and multidisciplinary rehabilitation. She arranges specialized assessments and gathers resources to meet our customers’ needs.

Administration

Niki McMillan, Clinic Administrator

Niki has been serving customers and assisting clinicians for more than 8 years at Park Street. As the clinic administrator she coordinates the business office functions and is our customer service team lead.

Annie Viala, Administrative Assistant

Annie assists our customers and clinicians by ensuring same day/next day access to treatment by arranging appointment schedules and keeping vital treatment information flowing between our clinic and customers’ primary care team.

Lindsay Wiskar, Administrative Assistant

Lindsay assists with coordination of medical records and reports for the specialized assessments we provide and providing same day/next day access to treatment by arranging appointment schedules.
