Occupational Therapy

Ian Lewis, Clinic Manager / Occupational Therapist

Ian joined Park Street in 2008 as Manager and provides leadership and clinical services. After graduation from the University of Alberta his professional experience has included practice in occupational health and safety, ergonomics, occupational rehabilitation and preschool/school age consultation. He has served and continues to serve in a number of roles on non-profit and professional boards. In his role as clinic manager and occupational therapist he hopes to enable people to do what they believed was not possible and achieve greater performance in life.

Physical Therapy

Lance Kirkpatrick, Physical Therapist

Lance has practiced orthopedic physical therapy for over 30 years and has practiced at the Park Street clinic for over 16 years. His extensive experience performing specialized assessments and keen interest in anatomy and biomechanics have made him a “go to” person for specialists and sponsors.

Raeleen Kirkpatrick, Physical Therapist

Raeleen has extensive experience in outpatient orthopaedics and community practice and more than 10 years practice with CBI. She has specialized training in acupuncture and manual therapy with more than 20 years in practice. She has a special interest in pain management. She is the clinical coordinator for multidisciplinary Secondary and Tertiary rehabilitation programs as well as the clinical educatior for the region.

Jordan Ruder, Physical Therapist

Jordan joined CBI after shortly after graduating with Distinction from the University of Saskatchewan School of Physical Therapy. He completed a post-graduate Diploma in Spinal Manipulation and FCAMPT designation through the Canadian Orthopaedic Division in 2014. Jordan's treatment approach combines manual therapy to maximize joint biomechanics with a significant emphasis on strength training as well as education to assist patients in achieving their athletic or functional goals. He enjoys treating active and motivated patients of all ages.

Andrea Wollbaum, Physical Therapist

Andrea completed a Bachelor of Kinesiology and Health Studies at the University of Regina with a double major in Adapted Physical Activity as well as Fitness and Lifestyle. She then went on the recieve a Master of Physical Therapy from the University of Saskatchewan. Andrea focuses on orthopedic and musculoskeletal disorders. She has been improving her knowlege and manual therapy skills in this area with continuing education opportunities, and is currently working towards her Diploma in Manipulative Therapy.

Dayna Onrait, Physical Therapist

Dayna completed a Bachelor of Science in Kinesiology from the University of Regina and a Master of Physical Therapy at the University of Saskatchewan. She started with CBI in 2012. Dayna provides physical therapy - including specialized treatments, education, exercise instruction and program coordination - in our primary program. She is currently completing post-graduate courses in the area of orthopedics and manual therapy.


Meghan Eashappie, Kinesiologist

Meghan graduated from the University of Regina with a degree in Kinesiology with a major in Fitness and Lifestyle.  Meghan also holds an Exercise Physiologist certificate from the Canadian Society for Exercise Psysiology. She holds Emergency First Aid CPR/AED HCP certification and has past experience working in cardiac rehabilitation.  Meghan currently works with clients in the secondary and tertiary level treatment programs.

Dr. Donald Kelm, Chiropractor

Dr. Kelm provides biomechanical treatment in our multidisciplinary rehabilitation program. He holds a Bachelor of Kinesiology and a Doctorate of Chiropractic, and also over 25 years experience as a chiropractor. He combines these two disciplines to deliver well-rounded care through a variety of myofascial and biomechanical treatments.


Michelle Shaw, Registered Doctoral Psychologist

Michelle has her Approved Practice Endorsement as a Registered Doctoral Psychologist and Neuropsychologist and has been practicing with CBI on a full-time basis since 2004. As a psychologist her primary focus has been the effects of anxiety and mood disorders on return to work and productive roles. She maintains a special interest in post-traumatic stress disorder. She provides psychological treatment on an out-patient basis as well as for clients in multidisciplinary rehabilitation programs. She also provides specialized neuropsychological assessment.

Pamela Clarke, Registered Doctoral Psychologist

Pamela has her Approved Practice Endorsement as a Registered Doctoral Psychologist. She has been practicing with CBI for more than 2 years and provides specialized treatment and assessment. She draws upon experience in community mental health and rehabilitation and sees outpatients and clients in our multidisciplinary rehabilitation programs.


Nicole Singh, Intake Coordinator

Nicole is available to answer sponsors’ and customers’ questions regarding our specialized assessment services and multidisciplinary rehabilitation. She arranges specialized assessments and gathers resources to meet our customers’ needs.


Niki McMillan, Clinic Administrator

Niki has been serving customers and assisting clinicians for more than 8 years at Park Street. As the clinic administrator she coordinates the business office functions and is our customer service team lead.

Annie Viala, Administrative Assistant

Annie assists our customers and clinicians by ensuring same day/next day access to treatment by arranging appointment schedules and keeping vital treatment information flowing between our clinic and customers’ primary care team.

Lindsay Wiskar, Administrative Assistant

Lindsay assists with coordination of medical records and reports for the specialized assessments we provide and providing same day/next day access to treatment by arranging appointment schedules.
